Since most information that a student might request in the Registrar's Office will require the staff to verify the person's identity, please provide a picture identification (e.g., driver's license) when requesting or picking up information requested in person.

CURRICULUM (MAJOR) CHANGES

Curriculum (major) changes are made by first contacting the respective department head or dean to obtain a curriculum change form. The completed form must be submitted to the Registrar's Office.

DIPLOMA REPLACEMENTS

To request a replacement diploma, please provide the following information in a letter with your signature.

1. The name under which you were registered and the name that was printed on your diploma
2. Your Social Security Number or ULM-assigned student ID
3. Your date of birth
4. The degree you received (e.g., Bachelor of Business Administration)
5. The college that granted the degree (e.g., College of Business Administration)
6. Month and year you graduated
7. The address to which the diploma is to be mailed

The cost for a replacement diploma is $45. The cost for a replacement diploma because you wish to order one with the new The University of Louisiana at Monroe name is $25. Please make the check or money order made payable to ULM. DROPPING / ADDING COURSES

Before adding or dropping a course at ULM, you should consult the current Schedule of Classes for add/drop dates.

ENROLLMENT VERIFICATIONS

Students often need to verify their enrollment status (either full- or part-time) for either insurance or employment purposes. Please visit the Registrar's Office to request a verification form.

FAMILY EDUCATIONAL RIGHTS AND PRIVACY ACT (FERPA)

ULM adheres to all requirements of the Family Educational Rights and Privacy Act (FERPA) regarding the release of academic records. Please visit our website which discusses and explains this Act.

COMMENCEMENT INFORMATION

Students who are approaching graduation should contact their academic department or college prior to the enrollment period in which they plan to be graduated. An Undergraduate/ Graduate Application for Degree must be completed and submitted to the Registrar's Office before a student is officially recognized as a degree candidate.

INTERNATIONAL STUDENT OFFICE

The ULM International Student Office welcomes international students to the campus. The social and cultural dimensions this group brings to our educational institution are immeasurable. International students who have questions regarding undergraduate admission may contact the Office of Admission at (318) 342-5430, while questions about graduate admission should be directed to the Office of Graduate Studies and Research at (318) 342-1036. International students who have questions regarding the campus and social life at ULM may call (318) 342-3678 or send e-mail to international@ulm.edu. NAME CHANGES

Students requesting a name change should contact the Registrar's Office regarding the documents necessary to make the change.

RESIGNATIONS

Students who need to resign from the University should consult the current Schedule of Classes for resignation dates and instructions.

ARROW REGISTRATION

Please consult the current Schedule of Classes for all information about ARROW Registration, including regular and late registration dates, registration procedures, and other instructions.

TRANSFER CREDITS

A prospective or already-enrolled transfer student should contact the respective department head or dean to determine which courses may transfer to ULM. Also, information regarding general education courses taught at other institutions in Louisiana which may transfer to ULM may be found at Louisiana Board of Regents Articulation System.

VETERANS AFFAIRS

The ULM Office of Veterans' Affairs assists veterans and their dependents in establishing eligibility to receive veterans' educational benefits. Requirements are that admission to the University is imminent, a DD-214 (military separation document) is submitted, and an application for veterans' benefits is completed, along with a statement of the tentative number of hours in which a veteran plans to enroll. Call (318) 342-5223 or 5224 for information regarding benefits.
